ABSTRACT: The study attempted to investigate the nurse-respondents perception of medication errors as to causation, identification, reporting, and safety precautions as a basis for the development of a proposed Safe Nursing Practice Protocol on Medication Administration (SNPPMA). Utilizing a combination of purposive and stratified sampling, and the descriptive method of research, 42 nurse-respondents assigned in the regular ward, intensive care unit and special areas of the National Kidney and Transplant Institute were asked to answer Modified Gladstone-Osborne (1999) questionnaire. The research instrument consists of five sections: demographic profile; and causation, identification, reporting and safeguard measures presented through scenarios and statements were respondents indicate their perception utilizing a four-point Likert Scale. Data gathered were presented using the frequency distribution; and analyzed using arithmetic mean, rankings, and Analysis of Variance based on a 0.05 level of confidence. Results of the study showed that majority of the nurse-respondents (73.81%) are staff nurses stationed in the regular ward. Among others, the study revealed that the prevalent causes of medication errors were technical and human-related and that there were varied perceptions on the identification of Total Parenteral Nutrition (TPN) feeding errors. Respondents strongly agreed that error reporting is valuable tool to measure medication competency. Findings, however, revealed significant differences in the perception of the respondents in terms of position. Application of the proposed SNPPMA that encompasses an action plan bolstered with the Decision Tree Model (Veloso, 2005) constituted one of the major recommendations of the study that aimed to curtal if not to wholly eradicate medication error incidence in a health care setting.
