ABSTRACT: In the course of practicing professional nursing in the workplace setting, the researcher had seen the importance of the role of health care professionals specifically, the nurses, in the bank institution. While the importance of the occupational nurse is well emphasized, promotion of healthy lifestyle among employees remains to be enhanced. Further exploration on healthy lifestyle and work productivity among employees is needed to develop a wellness program for the company. This undertaking was done to determine the extent healthy lifestyle and level of work productivity of employees. Specifically, it sought to answer the questions: profile of the respondents in terms of age, gender, position, and department, extent of healthy lifestyle on the following dimensions spiritual growth, interpersonal relations, nutrition, physical activity, health responsibility and stress management, level of work productivity in terms of attendance, quality of work, quantity of work and person factors, significant relationship between the subscales of healthy lifestyle and work productivity, significant difference in the healthy lifestyle and work productivity of employees when they are grouped according to profile variables and finally the formulation of wellness program for the bank employees based on the findings of this study. A survey involving 524 employees was conducted at Standard Chartered Bank. Data were analyzed using use of frequency and percentage distribution, weighted mean, cross tabulation, t-test for bivariate and ANOVA for multivariate. Results revealed that majority of the respondents belong to middle 26-45 years old, more female employees, officers dominates the population of the company and bulk of the populations belong to consumer banking. Respondents show moderate extent of healthy lifestyle in all subscale of healthy lifestyle. However, among the subscale health responsibility and stress management obtained the lowest general weighted mean. On the other hand, respondents show highly productive in all subscale of work productivity. It was found out that there is significant relationship between the subscales of healthy lifestyle and work productivity. Noticing, there is a significant difference in the healthy lifestyle and work productivity among employees when they are grouped according to profile. Significant difference in respondents healthy lifestyle in all subscale when they are grouped according to age was found. In addition, there is significant difference in respondents healthy lifestyle when they grouped according to gender in terms of spiritual growth, interpersonal relations, nutrition, physical activity and health responsibility. Similarly, it shows that there is significant difference in respondents healthy lifestyle when they are grouped according to position and department in terms of nutrition, physical activity, health responsibility and stress management. It was noted that there is significant difference in the work productivity of respondents when they are grouped according to age in terms of quality of work and person factor. Moreover, there is significant difference in the work productivity of respondents in all subscale when they are grouped according to gender. Likewise, there is significant difference in the work productivity of respondents when they are grouped according to position in terms of quality of work and quantity of work and person factor. Further, there is significant in the work productivity of the respondents when they are grouped according to department in terms of quantity of work. Finally, the findings suggest that the company implement the wellness program in order to promote healthy lifestyle and very highly work productive employees.
