ABSTRACT: This descriptive research work generally conducted to determine the significant relationship between the extent of spiritual needs and extent of spiritual activities of hospitalized older adults as a framework for wellness action plan. Specifically it sought answer to what extent do older adults express their spiritual needs in terms of understanding death, coping with suffering, moral values and transcendent love and to what extent do they perform spiritual activities in terms of personal and interpersonal aspect. Researcher-made questionnaire was used for the data gathering and the respondents were chosen via purposive sampling method. Weighted mean was utilized to determine the extent of expression of spiritual needs and performance of spiritual activities and the Pearson's correlation coefficient was used to determine if there is a significant relationship between these two variables. Data results showed that there was a very high extent of expression of the spiritual needs of the hospitalized older adults in terms of understanding death, coping and suffering moral values and transcendent love and there was a very high extent of performance of the hospitalized older adult's spiritual activities in terms of the personal and interpersonal aspects. It was concluded that there was no significant relationship between the extent of expression of the spiritual needs and extent of performance of the spiritual activities in terms personal aspect and interpersonal aspect except for the spiritual need moral values have a significant relationship with the spiritual activities in terms of interpersonal aspect . Out from these findings the researcher has developed a framework for a wellness action plan which includes that the nurse/caregivers and others of the older adults to work collaboratively as an advocate for the older adult's clinical, behavioral health, psychological, and spiritual needs and ensure that there will be a comprehensive assessment to identify their needs, an options for necessary services to be provided to them, and access to resources to meet their individual spiritual needs.
