ABSTRACT: STATEMENT OF THE PROBLEM: The study entitled “Parakalusugan ID: An RFID Technology Based Electronic Health Record Management System for the Out-Patient Department” is conducted to contribute to effective and efficient delivery of health services through appropriate and communication strategy, and to aid in health decision-making at the local level. This was conducted to find answers to the following questions: 1. Does the RFID Technology System help the user the following: 1. Keep track of their medical history 2. Allows rapid and effective Decision Making in treating the patient? 2.What are the advantages and disadvantages of ParaKalusugan ID in patient identification and verification? 3.What would be the challenges and obstacles in implementing ParaKalusugan ID in patient identification and verification? 4. In the overall experience in using the app user-friendly and easy to navigate? RESEARCH METHODOLOGY: The study, “Parakalusugan ID: An RFID Technology Based Electronic Health Record Management System for the Out-Patient Department”, used a descriptive-field research methodology. Descriptive-Field Research is a hybrid of the descriptive and field research methods that entails describing phenomena in order to describe “what exists” in relation to variables and circumstances in a setting. This approach is used to describe the process of adopting RFID technology in a healthcare organization in order to identify and explain the elements that impact the success or failure of the implementation in the field rather than in a laboratory or other controlled environment. Descriptive-Field Research is used in the research “ParaKalusugan ID: An RFID Technology Based Electronic Health Record Management System for the Out-Patient Department” to explore how developing a device might affect patient monitoring and diagnosis in the out-patient department. This allows researchers to collect data from various patients while keeping them in their manual environment. The patients were not subjected to any tests or stimuli that may affect their present health condition, and data was obtained from patients who were at ease and relaxed. The System Development Life Cycle (SDLC) was employed in this study. This is a traditional and methodical way to develop information systems used by system analysts. The researchers use the system analysis and design tools Entity Relationship Diagram (ERD) and Data Flow Diagram (DFD) to demonstrate a graphical depiction of the system that would be generated while developing a system. The Data Flow Diagram (DFD) is a graphical depiction of data flow across a process/system or subsystem/sub-process. It depicts what happens to the data as it passes through the process and use standard symbols and notations. A SDLC, or System Development Life Cycle, was essential in the development of systems, serving as a guide for developers in assessing the project. SUMMARY FINDINGS: The RFID technology based electronic health record management system has a device called ParaKalusugan ID that works with the developed web application. The ParaKalusugan ID web application was developed as a response to the need for improved access to healthcare services, particularly for patients who may have limited access to healthcare facilities or resources. The web application was designed to provide patients with easy access to their medical records and healthcare providers, allowing them to schedule appointments, communicate with their healthcare team, and receive timely and effective care. The primary objective of the ParaKalusugan ID web application was to make emergency care for patients as efficient and risk-free as possible by providing doctors with as much information about the patient as possible. The application aimed to improve communication between patients and healthcare providers, reduce the likelihood of medical errors, and ultimately improve patient outcomes. It was designed to be user-friendly and accessible, with a focus on patient-centered care. After different tests, interviews, and validations conducted the following finds were observed. 1. Surveys were administered to users to measure satisfaction with the ParaKalusugan ID web application’s features and functionality. 2. Patients and the head doctor of PLM’s university health clinic expressed high levels of satisfaction with the application, indicating its potential to enhance healthcare outcomes. 3. Patients found the application user-friendly and empowering regarding their healthcare, while the head doctor offered suggestions for improvement based on her preferences and requirements. 4. The study demonstrated the effectiveness of the ParaKalusugan ID web application in improving patient outcomes and satisfaction through improved access to healthcare services and patient empowerment. 5. The steps take to test this device was proven effective in storing health records of the patients. 6. Both patients and healthcare professionals can save a lot of time using ParaKalusugan ID than going to the hospitals or clinics to schedule appointments and access their medical records online. 7. Healthcare professionals were also able to reduce administrative tasks, such as managing medical records and scheduling appointments, allowing the to focus more on patient care. CONCLUSION: The ParaKalusugan ID web application, integrated with an RFID technology-based electronic health record management system, has proven to be effective in improving access to healthcare services, enhancing decision-making, and enhancing the overall quality of healthcare. The application serves as a centralized platform for securely storing and retrieving patient information, facilitating seamless communication and collaboration among healthcare professionals, and empowering patients to actively participate in their healthcare. The implementation of the web application in the Pamantasan ng Lungsod ng Maynila University Health Clinic has been successful, with high levels of satisfaction expressed by both patents and the head doctor. Patients found the application user-friendly and empowering, while the head doctor provided valuable feedback for further improvements. The application has demonstrated its potential to improve patient outcomes, satisfaction, and overall healthcare delivery. Through survey and feedback, it was observed that the ParaKalusugan ID web application effectively stores health records, saves time for both patients and healthcare professionals, and reduces administrative tasks. The application has proven to be a valuable tool in streamlining healthcare processes and enabling efficient access to medical records and appointments. The researchers came up with the following conclusions based on the findings obtained from the testing procedures conducted to attain the objectives of this study: 1. The study’s objectives of improving patient outcomes and reducing medical error were successfully achieved through the development and implementation of the ParaKalusugan ID web application. 2. The application provided patients with easy access to their medical information and communicating with healthcare providers. 3. The results of user satisfaction surveys showed high levels of satisfaction among patients, indicating the potential to improve healthcare outcomes. 4. The ParaKalusugan ID web application can serve as a valuable tool for healthcare providers in delivering high-quality care to their patients by improving communication and reducing medical errors. 5. The implementation of the device is accepted by the physicians and can be used to diagnose the patients. 6. The use of web application is proven as an effective means for physicians to check the medical history, diagnosis, findings, and assessment of the patient. 7. The device can also minimize the time and effort patients need just to bring their paper health record history taken by different clinics or hospitals. In conclusion, the ParaKalusugan ID web application, along with the RFID technology-based electronic health record management system, has shown promise in revolutionizing healthcare delivery by improving access, efficiency, and patient satisfaction. Further enhancements based on user feedback and continuous evaluation will contribute to its continued success in meeting the needs of healthcare professionals and patient alike.
