Abstract
The purpose of the study is to determine the relationship of treatment knowledge on the adherence of hemodialysis patients in AMOSUP Seamen's Hospital as basis for program development. The study involved the demographic profile, hemodialysis treatment knowledge, and hemodialysis treatment adherence of hemodialysis patients who are at least 18 years old to 90 years old are on hemodialysis for at least 6 months, are willing to participate the study. The study utilized a three-part survey questionnaire composed of the demographic profile questionnaire, the Chronic Hemodialysis Knowledge Survey (CHeKS) a 23-item questionnaire which assesses the knowledge of hemodialysis patients on selected issues including hemodialysis, nutrition, anemia, vascular access, medication, and safety (Cavanaugh et al., 2009)., and the End-Stage Renal Disease Adherence Questionnaire (ESRD-AQ) which adopted from Kim et al. (2010), a 46-item questionnaire which measures adherence behaviors and perceptions of patients on hemodialysis on 4 areas: hemodialysis treatment, medications, fluid restrictions, and diet recommendations. Statistical analyses were conducted using STATA MP-Parallel Edition Statistical Software, Version 18, College Station, TX: StataCorp LP. A p-value of 0.05 was considered statistically significant. Descriptive statistics included mean and standard deviation, frequency and percentage, and median and interquartile range to summarize and describe the sociodemographic profile, treatment knowledge, and treatment adherence among the respondents. Treatment knowledge was categorized as inadequate and adequate based on ≥50% correct responses (Cavanaugh et al., 2009). On the other hand, treatment adherence was categorized as non-adherent and adherent using median splitting approach, wherein scores with scores ≥50th percentile was classified as adequate and those <50th percentile were categorized as non-adherent (Lautert, 1997; Pera & Serra-Prat, 2002). A total of 55 hemodialysis patients were gathered as participants. The study findings revealed that the mean age of the participants was 55.78 years old, most of them were females, were married, had completed tertiary education, and earned more than PHP 20,000.00 per month. Regarding treatment knowledge, the dimensions of hemodialysis and nutrition had the highest mean percentage correct responses, while the mean percentage of correct responses to the dimensions of Vascular access, safety, psychological status and medications was the lowest. Among the four dimensions of treatment adherence that were tested in this study, it shows that most patients were adherent to medication and fluid restriction. But despite of showing the same results (adherence) on all of the dimensions, overall Treatment Adherence was reported to be Non-Adherent (50.91%). Compliant to prescribed treatment regimens in hemodialysis can lead to effective disease management, thus patients should adhere to maintain optimal health. The results indicated that none of the sociodemographic characteristics were significantly associated with treatment knowledge. Marital status, educational attainment, socioeconomic status, and duration of hemodialysis treatment were not statistically and significantly associated with treatment adherence. However, results showed that both age and sex were significantly associated. Results for age indicated that with every one-year increase in age, the probability of being adherent to treatment increased by seven percent. On the other hand, results showed that the female sex has an inverse relationship with treatment adherence. This result denotes that the likelihood of treatment adherence is four point seventy-six times compared to males. This result also denotes that patients with adequate knowledge are five point fifty times more likely or at a higher probability to become more adherent with treatment compared to those with inadequate treatment knowledge.
