Action note: In: Summary: ABSTRACT: The study aimed to to determine the respondents developmental profile according to physical, cognitive, affective, social, and spiritual aspects, and to identify the risk behaviors as a basis for developing a wellness program. Utilizing a combination of purposive sampling and descriptive method of research, seventy six respondents are asked to answer a modified Katz model of assessment for the physical aspect of development and researcher-made questionnaires for affective, social, and spiritual aspects. The research instrument consists of two sections, developmental profile and risk behaviors, where respondents indicate their responses utilizing a three-point Likert Scale for the physical aspect of development and a five-point Likert Scale for the affective, social and spiritual aspects, and for the health behaviors. The data gathered were analyzed using weighted mean and Spearman Rank-Order Coefficient Correlation based on a 0.05 level of significance. Results of the study showed that the respondents have evidences of decline in physical, affective, social, and aspect of development. As to the spiritual aspect, signs of increased level of inner strength in coping to changes losses and stresses were noted. Moreover, risk behaviors were identified. Findings revealed that there is significant relationship between and among the developmental profile and risk behaviors of the elderly. Application of the proposed wellness program constituted one of the major recommendations of the study that aim to facilitate functional independence in the elderly. Other editions:
