_aABSTRACT: This study was conducted to assess and correlate the level of knowledge and practice of hypertensive community residents to be able to formulate a community lifestyle program in accordance with the profile, level of knowledge regarding hypertension and level of practice regarding management and treatment of hypertension. Also, significant difference on the level of practice of the respondents regarding management and treatment hypertension when they are grouped according to profile was measured. Furthermore, to determine the significant relationship between the level of knowledge and level of practices of the respondents regarding hypertension. A descriptive study through purposive survey was utilized in the study. The study was conducted in Barangays 707, 712, and 714 in Zone 78, Fifth District of Manila. Purposive sampling was utilized in the study in which the researchers went house to house in a span of one week. A self-made tool was utilized to collect data from the respondents, which was validated by a cardiologist, a general practitioner and a nurse supervisor in a medicine ward. The questionnaire was composed of three parts, the first part was to group the respondents according to profile, the second part aimed to assess the level of knowledge of the respondents about hypertension, the third part aimed to assess the level of practice of the respondents regarding their illness. The statistical treatments used for the study were frequency and percentage distribution, weighted mean, Pearson r, and ANOVA. The findings of the study were: The majority of the respondents belong to the middle adult age group, female, high school graduate/undergraduate and has a daily individual income of below 429 php. There is a significant difference between the level of practice regarding management on hypertension when grouped according to the age grouped were the respondents belong. Therefore, there is a significant difference between the level of practice regarding management and treatments of hypertension when grouped according to the sex were the respondents belong. In contrary, there is no significant difference between the level of knowledge on hypertension when grouped according to the sex were the respondents belong. There is no difference on the level of knowledge on hypertension when grouped according to educational attainment. Also, there is no significant difference on the level of practice regarding management and treatment of hypertension when grouped accordingly to daily individual income. The recommendations were: 1) The researcher recommend that the hypertensive residents participate in the activities proposed in the program for them to be able to have better knowledge and health practices on their illness. Also for them to have improved conditions about their illness. 2) The researchers recommend that the community residents participate in the activities of the program for them to be able to have better understanding about hypertension and in turn, prevent the development of the illness. 3) Researchers recommend the Barangay Officials that they use the results and the output in implementing different health programs that will be catered to their community. 4) The researchers recommend the health care providers that they use of this research and its output as a guide in formulating appropriate programs for the residents in the community. 5) The researchers recommend to the future researchers to get a larger population of the respondents. In addition, the researchers also recommends that the research be conducted in a different locale to test the study's consistency and applicability on different populations.
